Electronic Records and Signature Disclosure

1. Agreement to Electronic Transactions
When you select an electronic-consent checkbox, adopt an electronic signature or initials, or complete a OneagencyOS signature packet, you agree that the sender and you may use electronic records and electronic signatures for that transaction. An electronic signature has the same intended effect as a handwritten signature. Your consent applies only to the documents and notices described in the packet or related transaction unless you separately agree to broader electronic delivery.
2. Documents Covered
Covered records may include agency subscription agreements, order forms, data-processing addenda, insurance applications, disclosures, authorizations, acknowledgments, policy-service forms, cancellation or renewal forms, invoices, client-portal notices, and related amendments or copies. Certain records may be excluded when law requires another method or the sender determines that paper or in-person delivery is appropriate.
3. Right to Paper and No Obligation to Consent
You are not required to use electronic records or signatures. Before signing, you may ask the sender to provide a paper process. After signing, you may request a paper copy of the completed record. OneagencyOS does not charge a fee for an electronic or paper copy, but the sender may disclose a reasonable mailing or processing fee before fulfilling a special request.
4. Withdrawing Consent
You may withdraw consent for future electronic records in the covered relationship by contacting the sender at the email shown in the packet or contacting OneagencyOS at hello@oneagencyos.com. Withdrawal becomes effective after the request is verified and processed and does not affect the validity of records or signatures completed before withdrawal. Withdrawal may require future transactions to be completed on paper and may delay service.
5. Hardware and Software Requirements
You need a current email address; internet access; a current version of a commonly used browser with cookies and JavaScript enabled; a device capable of displaying HTML and PDF files; and the ability to download, save, or print records. A PDF reader may be needed for downloaded documents. If these requirements materially change in a way that creates a significant risk you cannot access covered records, the sender or OneagencyOS will provide updated requirements and, when required, obtain renewed consent.
6. Keeping Contact Information Current
You must provide an accurate email address and promptly notify the sender when it changes. A notice sent to the last email address provided may be treated as delivered to the extent permitted by law. If delivery fails, the sender may contact you by another available method.
7. Demonstrating Ability to Access Records
By opening the signature packet, reviewing this disclosure, and completing the electronic-consent step, you demonstrate that you can access the electronic information in the format presented. Do not consent or sign if you cannot view, download, or retain the documents.
8. Signature Attribution and Audit Trail
OneagencyOS may record the signer's name, email address, authentication event, consent, document version, field placement, timestamps, IP address, device or browser information, delivery events, open events, signature and initial events, completion status, and completed document hash. These records help attribute the signature, protect document integrity, and create an audit certificate. The sender is responsible for selecting an identity-verification method appropriate to the transaction.
9. Downloading and Retaining Copies
Review the entire packet before signing. After completion, use the provided download or print control to retain a copy. The sender may also email a completion notice or completed copy. If you cannot retrieve a copy, contact the sender or hello@oneagencyos.com.
10. Insurance Transactions
Electronic-signature availability does not change insurance coverage, binding authority, carrier approval, premium payment, effective dates, or other substantive requirements. A signature does not bind coverage unless an authorized insurer or producer separately confirms binding under applicable rules.
11. Consent Statement
By selecting "I agree," "Consent," or a similar control and applying your electronic signature or initials, you confirm that you have read this disclosure, can access and retain the records, consent to the electronic transaction, and intend your electronic action to serve as your signature.
